Domestic Violence
Cross Government Definition of Domestic Violence:
"Any incident or pattern of incidents of controlling, coercive, threatening behaviour, violence or abuse between those aged 16 or over who are or have been intimate partners or family members regardless of gender or sexuality"
The definition also includes 'honour' based violence, female genital mutilation (FGM) and forced marriage.

National Domestic Violence Helpline
The 24hr freephone National Domestic Violence Helpline (run in partnership between Women’s Aid and Refuge) is available 24 hours a day, 7 days a week.

Coordinate My Care (CMC)
Coordinate My Care (CMC) is an online personalised care plan accessible to healthcare providers (including The Vale Practice, the ambulance services, out of hours GPs, hospital staff and people providing social care).
This allows you to express your wishes about your care and to help the urgent care services to know more about you if you need them. You can let us know your wishes regarding where and how you wish to be treated depending on how unwell you are, who we should contact in an emergency and even what you would want if your condition was to get worse.
Coordinate My Care ensures that you get the care that you want, in the place that you want. We feel that it is important that any views about your future care that we might not yet be aware of are recorded properly.
If you think a CMC plan might be of benefit to you or to your loved one, please get in touch with the practice and ask to speak to a doctor or nurse about it.
